UC forms

Does anyone know where I can get a copy of some UC forms? In particular I’m looking for G4 and S4. Can they be downloaded? I can’t find anything online.

I’m not aware of these forms, however a quick google has supplied this website:

I can’t see for certain as I don’t have a registration with this site, but it appears that they may be template letters created by the website, and so not actually sourced from DWP.

Thanks MareeH, That explains it as the form has apparently come from a Housing Association.
