Examples of DHP forms needed

Hi all,
We’d like to gather together some examples of DHP forms - particularly any that are hard to complete, ask irrelevant questions or have overly demanding evidence requirements - so any examples you have would be very welcome,

Hi, we deal mainly with 3 local councils and were just looking at these last week and I have to say it’s great to have such a varied selection of application methods!
Wandsworth - apply by letter

It normally takes me 1 -2 hours to complete. It once took me 3 hours!
I think it asks many unnecessary questions.
Chesterfield has some strange questions…or bad grammar (question 2) and wants lots of proof. As a mental health team we actually have a great response from them and they’re really helpful…their form doesn’t reflect this.

Tell a lie…. the Reading Borough Council DHP form I uploaded was the old one. I have now attached the current one (still fairly user-friendly)
