PIP - assessor not able to access CQ or FME

Ive just had a claim declined with no points as the assessor was not able to access any CQ or FME.  Claimant has serious mental health issues and was not as frank with the assessor as she was with me when I completed the CQ with her.  Im going to do a Mandatory Recon and then probably an appeal.  Does anyone have any experience in dealing with claims without the information being available to the HP ??  Many thanks
