Contributory ESA award letter

Has anyone got a contributory ESA award letter they could redact and scan to me? Or a sample?

I’m trying to argue that an NHS penalty charge should be lifted and what a nightmare the letters are is key but I don’t have one to hand to illustrate it. The bloke who’s been hit with the charge is not brilliant with his correspondence so I hold little hope he’s kept his uprating notice.
