Hi

I seem to remember there being a complex needs form or something to add to UC journal. Has anybody got a copy?

This is the form developed by Steve Johnson CPAG trainer. DWP don’t really know what the form is for, so make sure to flag up that it is to notify of Complex Needs and request that it is pinned to top of notes as Work coaches can pin important notes on their screen.
