Challenging Continuing Health Care Decisions

Are any other Local Authorities and Welfare Rights Units looking at challenging or currently challenging continuing health care decisions.  We had one recently which resulted in a cost to our Department that was arguably not our responsibility and Gentleman and his Family incurred a charge for care at home ( Teminally ill Gentleman with weeks to live sent home and cost was not picked up by Health Authority). I accept time scales were very short but there may be scope for considerable savings for Local Authorities and the Families affected if more decisions were challenged. 

Any thoughts on this please?  What is the policy elsewhere?  Who checks and challenges these decisions?
