PIP review - award removed via telephone assessment, no review form issued

Hi Everyone,
I have a client who has just had her PIP award removed at a review. She was never asked to complete a review form, but was contacted by the assessment provider to arrange an appointment. Client initially though that the person was going to complete the review form with her over the phone, but it was a telephone assessment. I am going to assist with an MR on the basis that her functioning has not changed and to reinstate the award, but has anyone else come across this situation before?
