DLA to PIP transition - administration issues meaning PIP2 not issued

Had not previously experienced this so thought would flag:

Claimant on DLA invited to claim PIP - rang the helpline. 

15 days later PIP2 not received in time for our appointment to complete.  As this is overdue I assume lost/delayed -  claimant is off on holiday and worried it would come whilst away/miss deadline etc we advise to ring PIP helpline again. They advise claimant “there was a computer error and it was never sent out.” Will send it.

First time we’ve had “computer says no” to actually issuing the form - and claimant tells me they also pointed out at first phone call that the PIP letter was addressed to claimant in maiden name.
Claimant’s DLA claim was in married name throughout (Married >20 years, DLA >10 years)

No idea what would have happened if PIP2 form not issued and claimant did not enquire - a further invite to claim? DLA stopped for failure to return an unissued form?

I’m intrigued as to the processes by which these errors arose and how did the transfer of information from DLA to PIP result in invitation to claim letter in wrong name?
