Call for evidence - the vulnerable & sanctions

Is anyone gathering evidence of vulnerable service users being sanctioned inappropriately?

The guidance seems to indicate that there are safeguards in place to prevent this happening but my suspicion is that this guidance is routinely ignored.

Individual case studies or wider quantitative evidence would be helpful to persuade a Court that this is an issue impacting a group of people rather than a lone bad decision in respect of an individual claimant.

I am conscious that I might have missed some sources for this so any help/guidance would be much appreciated.
