Colostomy Bag and Catheter for PIP- activity 5

I have a client who wishes to apply for PIP.

Due to bowel cancer which has now been treated and is remission he has applied to PIP as he uses a Colostomy Bag and Catheter and is totally dependent on it. He states that he does not have any other conditions apart from this- would this score him potentially 2 points (as he uses an aid) or 8 points as he does not actually need someone else to help him with this and is able to complete the activity himself but he does have incontinence with his bladder and bowels? Am I missing something?

Are there any guides to help complete a PIP form with someone with incontinence?

I was thinking that Activity 3: Managing therapy or monitoring a health condition as he uses an aid- a colostomy bad and catheter and Activity 9: Engaging with other people face to face may also apply as he states he is very nervous and anxious going out to places and being around people he does not know due to anxiety of having an accident or the bag making noises- which they do at times and he doesn’t use public transport as a result of this.

May also be worth drilling down further into symptoms of fatigue if they apply to the other descriptors.
