Here is the DWP’s sample version of the new PIP2 (I put in a FOI request before I’d seen the copy of the form given previously in this thread).

At the stakeholder forum last week we were told that the online PIP2 should be available for anyone that wants it from June 2021…

is the form now an online form (rather than a PDF)?. And, does anyone have a mock up of it?
