Email from DWP -

For Information- for those of you with a gov.uk email address who support clients with a terminal illness

DS1500 – Notification of Terminal Illness Form Changes

For the attention of staff in Local Authorities involved in benefit advice or similar work.

In light of the difficulties faced by many working from home DWP has opened up an email facility to allow the DS1500 to be emailed to the Department rather than needing to be printed and posted in. The DS1500 will only be accepted if it sent in from a .gov.uk email address – forms received from personal or non .gov.uk email addresses will not be actioned. We already have a facility to receive the DS1500 from NHS.net and Wales.nhs email addresses.

The DS1500 should be sent to the following email address: .(JavaScript must be enabled to view this email address) (link sends e-mail)

has anyone heard of or had success in sending DS1500’s for AA electronically?
Our CNS’s working remotely are having to send in ‘paper’ copies, causing delay to AA claims for terminally ill patients
currently use our .nhs emails for PIP e-DS1500’s with great success

Thank you.

Would that be for DLA and AA?

Thank you - as ever

We have that email address for PIP but need the equivalent for AA. Anyone even got a direct tel number for SRTI AA teams? Covering London and South East mainly but happy for anything right now given the limited service being offered according to the recorded message .

We have that email for PIP DS1500 but also looking for same for AA. Has anyone even got a tel number for AA SRTI teams? We are covering mostly London and South East but will take for anywhere given the very limited service tel message is informing us about.

I emailed the DWP to see if there was an equivalent email for DS1500s for DLA, AA, ESA and UC - this is the response -

The email facility at the moment is just for PIP DS1500’s but if it becomes available for other benefits I will of course let you know asap.

I have been using this e mail address for both PIP and AA electronic DS1500’s for several months now without any issue. They just pass the DS1500 to the AA section which is then married up with the AA claim form which I follow up in the post.
