Safeguarding vulnerable client’s / Complex needs alert proforma thingy in word

Colleague Sarah found the attached posted on CITA’s Friendface HTC bit - Provenance is not clear but there is a reference on Facefriend to a freelance CPAG trainer (no name).
Its in word form so can be copied and pasted on to journal as well client handing a copy to WC’s.
I say copied and pasted because Sarah says journal entries are limited to 3000 characters, the attached has Characters (with spaces) 2,925 and characters (no spaces) 2,546.
Arguably, the above has multiple uses other than the obvious e.g. when we print off the journal contents for appeals for Sanctions and perceived failure to agree claimant commitments (auto generated).

Is there anything stopping the use of this template for other benefits (i.e. different guidance for defining vulnerability etc.)? Or are there more appropriate templates for other benefits?

I’m too brain dead to offer a coherent answer at this stage of the week, but see attached JSA template
has been posted on Rightsnet a few times from Wales via Suffolk.

The Complex Needs Alert form was devised by Steve Johnson who works freelance for CPAG. We have been using it with our Complex Needs customers for a few months and it seems to be helping to highlight potential issues to UC. Ideally it can be used to inform them of a customers vulnerabilities at a Verification Interview or at a further meeting with a Work Coach.
