Help! - Comm/UT decision - benefit office record keeping is not infallible

Help please!

I have in mind that here is a Comm/UT decision in which the judge says it is not his/her experience that benefit offices record everything and are infallible - refuting the submission routinely made by DWP/HMRC/HB that they record (in case notes and/or a voice recording) all telephone calls and attendance in person at the benefit office and therefore if there is no record the claimant did not contact them.

I also have in mind that it was a HB decision (but will turn out to be TC or IS!!).
