This was alluded to on Mr Donnison's Benefits and Work website a couple of days ago and now I have had one forwarded to me by a client.

It's called 'Renewal Test June 2007 01' and asks 2 closed questions about mobility & care needs, one about any 'non-disability' changes in circs and then asks if your phone number has changed.

If there are no changes to report then it's 4 ticks and a signature.

Good if you are content with your current award I guess. But wither the argument that each award of DLA is a separate exercise in establishing entitlement I wonder?
