Lost date stampted AA form

Hi all,

This is a total shot in the dark but here goes.....

Client ordered a date stamped AA form for his wife for a claim under the special rules. The claim form was date stamped from 05/11/09 and to be returned within 6 weeks. The client gave the form to the person issuing the DS1500. Unfortunately, the person issuing the DS1500 got confused and thought the AA claim form was not needed anymore (as they had the DS1500) and threw it away.

As far as I can see, there is no way I can get the claim to be started from 05/11/09 due to the lost form? Does anyone know if DBCs keep a record of date stamped claims?

I ordered a new pack today but we are still looking at a shortfall of a few weeks.

The client is only expected to live for the next couple of days. Given the nature of the client's health the family are very distressed about the situation. Any help would be gratefully appreciated.

Hi

The DBC customer service manager has previously confirmed that they do not keep records of who ordered form and when, but don't despair I would suggest writing a letter to acompany the new completed claim form explaining the confusion by the DS1500 issuer and ask for compassion. Given that this is a special rules case they are likely to be flexible on this issue and I have yet to have this sort of request refused.

I would agree with the above and whilst records of requests for claim forms are not specifically recorded, i have absolutely no doubt that if the form was ordered in the name of the claimant and a NINO provided, that there will be a trace that a form was in fact requested, it may not state the date of the 5th, but it should have a trace of the date of the call.
