Because of staff shortage we no longer assist to fill in forms for DLA. With my clients I provide a spare DLA form as draft and a checklist. Draft is completed and checked and then copied to main form . Its working well and has bonus in the client is actively involved. I've got an adult client with OCD and would be grateful if anyone has an OCD checklist. The general checklist, although ggod, doesn't cover sufficiently for her condition.
