'Extreme' behaviour for HRM on SMI grounds

Does anyone have link to the full text of CDLA/2054/1998 or any other decisions that give any authorative view of the term 'extreme' . I've got R(DLA)7/02 but all that does is quote (with approval) the decsion above and neither Mark Perlic's excellent book or the commentary in Bonner cite any others.

This is a classic commissioner's decision: that is, it makes no findings of fact but sets out clearly what factors should have been investigated, and are capable of amounting to extreme behaviour. As so often (indeed, almost invariably) every case will turn on its own facts, but the principles to be applied will be the same. Another case might turn up a different behavioural pattern and the commissioner's guidance will then have to be applied and adapted accordingly.
