Which consent form...

...is needed by the TCO for them to disclose claimants details. My colleague is the client's Receiver as appointed by the Court of Protection.

I can't seem to find anything by a web search. The helpline has suggested form 64-8 but it doesn't seem appropriate.

The only two forms I know of are the 64-8 and the TC689. There is some information in the online manuals (see link below) that suggests that the contact centre should make a referral.
