I have been asked if when you reclaim within the 13 weeks period do you have to complete a "new" claim form. This client went back to work for 5 weeks but was not ready so has been signed off sick again. She rang DWP and was sent a ESAT form (50 pages long). She is only entitled to contributory ESA. I have looked through the DWP guidance but can find no reference to a short renewal claim form. Looked to see if it can be done online. no joy. Is there a short renewal claim?
