DS1500 - Copy of form or link to download.

Hi

Does anyone have a pdf of the DS1500 they could let me have or a link to the form on-line.

I have been unable to find the form when I searched for it.

This link to the Gov website for AA says you can only get a DS1500 from doctor, specialist or consultant, which would make sense as it’s not a claim form in and of itself.

The DS1500 is a bit like a prescription pad - access to the original forms (ie with no writing on) is limited to medically qualified folk.  If you just need to see one I can anonymise one and send it to you through email though.  The pads don’t seem to have been updated to include PIP yet.

Margaret, Julie is correct. They are held by GPs, consultants or Clinical Nurse specialists. I do hold a pad of them because sometimes a consultant / CNS doesn’t have any and it saves me time (I am based within the hospital). They need to be ordered from the iOn contact centre by a medical professional on headed paper from the practice/hospital.

Yes, they’re restricted. I have been accidentally sent pads in the past though. There are some that have been updated for PIP, but whichever form you have will be valid for the claim.
