Examples of DHP forms needed

Hi all,

We’d like to gather together some examples of DHP forms - particularly any that are hard to complete, ask irrelevant questions or have overly demanding evidence requirements - so any examples you have would be very welcome,

Hi, we deal mainly with 3 local councils and were just looking at these last week and I have to say it’s great to have such a varied selection of application methods!

Chesterfield has some strange questions…or bad grammar (question 2) and wants lots of proof. As a mental health team we actually have a great response from them and they’re really helpful…their form doesn’t reflect this.

North East Derbyshire. Pretty much the same form as Chesterfield with their NE logo. I guess they worked together as are based in the same town. . http://www.ne-derbyshire.gov.uk/advice-and-benefits/benefits/discretionary-housing-payments/
