ESA ‘Customer Statement’

Has anyone got a reference number or a copy/scan of an ESA ‘Customer Statement’, the one that is sent to the claimant for checking just after the claim is phoned in. I’ve searched the DWP website for a specimen version but without a reference number I haven’t yet had any luck.

Andrew, out of interest does your Customer Statement start at Page 5? Do you or anyone else know what Pages 1-4 contain?

Page 1 (of 18) says thank you for claiming but we need more information - it also advises to claim CT help -
page 2 gives instructions as to what evidence should be obtained, by when and where to send it - (it also says do not sign or return the Customer Statement) -
page 3 gives anti-fraud and naughtiness warnings and what will happen about the decision (plus a number to call) -
page 4 is ‘how we collect and use information’
