ESA repeat claims - form asking for details about new condition / deterioration
“We are also introducing a new form asking claimants for more detail on how their health condition affects them differently to when they were last assessed. This should help to ensure that decision makers will have the evidence they need to determine whether the claimant can be treated as having LCW (pending a new WCA).”
^^Does anyone know what this form is called, or even better have a copy?
