Claiming severe disabilty premium

It used to be that to claim the severe disabilty premium/addition that form IS10 or PC10 was completed. Understand these are no longer availible - would appreciate advice on the easiest way people can claim? Do people have to contact the JCplus directly? Thanks.

JCP and the Pension Service still issue IS10/PC10s. Anecdotally, I was told by a TPS adviser that often the information is gathered by telephone call for speed.

As Edmund says - as far as I am aware, IS10 and PC10 forms are both still being sent out on request.

We have copies of both in the office - can scan one in as a PDF if you like?
